New Student to SRMS?
Are you new to Sunset Ridge Middle School? We need: a withdrawal slip (if enrolled in a school currently), Birth Certificate (original), Immunizations (must be up to date, list of immunizations that are needed for middle school are located in the Counseling Center tab), Proof of Address (click below for form and a list of what is accepted.)
